ACA JAX Health Insurance is operated by AM CONCEPCION HEALTHCARE LLC, an independent health insurance agency located at 13111 Atlantic Blvd, Unit 1, Jacksonville, FL 32225.

AM CONCEPCION HEALTHCARE LLC is an independent health insurance agency.
ACA JAX Health Insurance is not HealthCare.gov, the Health Insurance Marketplace, the Centers for Medicare & Medicaid Services (CMS), or any other federal or state government agency.
Nothing on this website should be interpreted as indicating that the United States Government, the State of Florida, CMS, HealthCare.gov, or any other government entity endorses, guarantees, sponsors, or operates ACA JAX Health Insurance.

Eligibility for Marketplace financial assistance is determined under applicable Affordable Care Act and Health Insurance Marketplace requirements.

7. Website Inquiry Is Not Insurance Enrollment
Submitting a form through ACAJAXHealthInsurance.com does not enroll you in health insurance.
Submitting an inquiry also does not guarantee that coverage will be issued or that an insurance application will be approved.
Insurance coverage becomes effective only after all applicable enrollment requirements have been completed and the Marketplace and/or insurance carrier confirms enrollment and coverage.

8. Authorization for Marketplace Assistance
Submitting a general website inquiry or requesting information is separate from authorizing an agent or broker to assist with a Health Insurance Marketplace application or enrollment.
When required, ACA JAX Health Insurance will obtain and document the consumer’s consent before providing Marketplace enrollment assistance or facilitating enrollment in accordance with applicable CMS requirements.
Additional authorization, identity verification, application review, confirmation, or consent documentation may therefore be required before an ACA JAX agent can submit, modify, or facilitate a Marketplace application or enrollment on a consumer’s behalf.
Consumers may withdraw applicable Marketplace assistance consent in accordance with the consent process provided to them.

9. Contact Requests
When you voluntarily provide your telephone number and request assistance, you authorize AM CONCEPCION HEALTHCARE LLC and its authorized representatives to contact you by telephone regarding your inquiry and health insurance options, subject to the consent language presented when you submit the applicable form.
Providing consent to be contacted is not, by itself, an agreement to purchase insurance.
ACA JAX Health Insurance does not currently advertise text messaging as a consumer contact option through this website.

10. No Professional Tax or Legal Advice
Information provided through this website or by ACA JAX Health Insurance should not be considered legal, tax, accounting, or financial advice.

ACA JAX Health Insurance cannot guarantee:
